Career Samplers - Summer Camps - Participant Information Form

Participants must be registered via web registration or register in person at The Hub — Confederation College (Shuniah building), 1450 Nakina Drive, Thunder Bay.  In order to finalize your registration, complete this form.

Personal information on this form is collected in accordance with sections 21, 39 and 43 of the Freedom Of Information And Protection Of Privacy Act and under the legal authority of the Ministry Of Training, Colleges And Universities Act, R.S.O. 1990, and the Ontario Colleges Of Applied Arts And Technology Act, 2002, Regulations 34/03, and will be used for administration of the program, marketing and in cases of an emergency.

Anaphylactic Allergy
Please specify if your child has any allergies, medical conditions, disabilities, identified needs or behaviours that we should be aware of that require special consideration. If your child has an anaphylactic allergy, the coordinator will call to discuss procedures. We cannot guarantee an allergy free environment. Note: Participants requiring one on one attention are expected to bring a support person to the camp each day. Please advise staff at time of registration.
Agreements & Consents

I hereby agree to abide by all rules and regulations of the Career Samplers for Youth program (“Career Samplers”). I understand that if I break any of the rules, I will be asked to leave Career Samplers without the possibility of refund of fees. I am aware that breaking the rules and regulations may result in my exclusion from any future Career Samplers programs. I also agree to abide by and respect all decisions and actions of the Instructor and any staff member of the Confederation College of Applied Arts and Technology (“Confederation College”). I understand that Confederation College is not responsible for any lost or stolen property, nor for any injury including death sustained at Confederation College. I am confident that I can uphold exemplary behavior through my Career Sampler.

 

I hereby consent to the above idenfified participant's enrolment in Career Samplers. I understand and consent to the conditions of enrolment as noted above. In addition, as parent/guardian of the above named participant, on my own behalf and on his/her behalf, I hereby agree to release, indemnify and save harmless Confederation College,  its Board of Governors,  officers, employees, representatives, agents, assigns and licensees, as well as any other government ministry, agency, organization, company, or individual directly or indirectly connected with Confederation College or Career Samplers , from any cause of action, claim, lawsuit, charges, damages, demand, loss, cost, expense or judgment, resulting in any injury including death, or damage to property, that may be caused by the negligent or deliberate acts or omissions of the above named participant, or sustained by the above named participant, while engaged in any activity connected with Confederation College or Career Samplers, whether on or off the Confederation College premises.  Entering my name above indicates my understanding and acceptance of this waiver and consent.

I understand that there are inherent risks associated with the publication of photographs or videos of the participant by Confederation College, whether known or unknown. I understand the above mentioned risks and give permission for photographs and videos to be taken of the participant, while participating in Career Samplers at Confederation College. I understand these photographs or videos may be used for current and future publicity purposes, and I agree to such uses.

I hereby authorize the staff of Confederation College, to act for me according to their best judgment in any circumstances in which the participant may require medical attention, and I hereby waive and agree to release, indemnify and save harmless Confederation College, its Board of Governors, officers, employees, representatives, agents, assigns and licensees, from any and all liability in relation to his/her receiving the above mentioned medical attention. I realize that such medical attention may include but not be limited to first aid procedures, and attendance with the participant at a hospital.

I (parent/guardian) give permission or do not give permission (please check one) to leave the Career Samplers for Youth site at Confederation College during lunch with the understanding that should my child be late or not return I will be notified immediately at the phone numbers provided. I agree that Confederation College shall not be responsible for any injury including death, or property damage, sustained by the participant during his/her absence from the site.

Note: It is the responsibility of the participant to personally inform the Career Sampler Instructor prior to leaving.

Registration takes place on a first come, first serve basis. Confederation College reserves the right to cancel or alter courses as deemed necessary at which time we will make every effort to contact you via telephone and/or email. You will have the choice to register in another course, if applicable, or receive a full refund. A full refund less a $20 administrative fee will be issued if withdrawal request is received prior to final withdrawal date. Any refund requests after the deadline will not receive a refund unless the seat can be filled. No refund will be issued after the first scheduled class has begun.
