In Case of Critical Injury or Fatality (SAFE Campus)

In Case of Critical Injury or Fatality:

 “Critical injuries”, as defined in the Critical Injuries Regulation, made under the Occupational Health and Safety Act, include injuries that:

  • place life in jeopardy
  • produce unconsciousness,
  • result in substantial loss of blood,
  • involve the fracture of a leg or arm but not a finger or toe,
  • involve the amputation of a leg, arm, hand or foot but not a finger or toe,
  • consist of burns to a major portion of the body; or
  • cause the loss of sight in an eye.

All critical injuries (or fatalities) that occur on campus, or arise in the course of College-related activities, must be reported to the Ministry of Labour. Reporting is initiated through Human Resources Services—Health & Safety.

In the case of a critical injury, or fatality, the following will occur:

The Staff on Scene (or Security, as applicable) will:  

  • Provide necessary first aid / health care
  • Arrange transportation to a medical facility
  • Secure the scene of the accident using barricades, caution tape, signage, etc.. (The scene of the accident must not be disturbed or altered, except to save a life, relieve human suffering, or to prevent unnecessary damage to equipment or property. The accident scene is maintained until authorized by the Ministry of Labour, and such other agencies as are involved, to release it.)
  • Obtain the names, addresses and statements of witnesses
  • Ensure, in the case of a fatality, that the Police have been promptly contacted through the 911 Dispatcher.
  • Immediately notify the Manager, Health + Safety.

The Manager (Health + Safety) will:

  • Ensure that the Ministry of Labour Health and Safety Contact Centre has been promptly contacted at 1-877-202-0008) and that as many of the basic facts of the occurrence as are known have been provided:   
    • Name and Address of the Person Who Was Critically Injured (or Killed),
    • Social Insurance Number,
    • Date of Birth,
    • Date of Hire,
    • Start Date of Current Position,
    • Current Job Title, Department,
    • Supervisor's Name,
    • Name & Address of Employer,
    • Time and Place of Occurrence,
    • Name and Address of Physician or Surgeon, if Any, by Whom the Person Was or Is Being Attended for the Injury,
    • Nature & Circumstances of Occurrence & Bodily Injury Sustained;
    • Names of Witnesses to the Occurrence.
  • Notify a certified JHSC worker representative and a certified Management representative of the applicable Joint Health and Safety Committee (JHSC) to commence an investigation.  (The JHSC investigation report will include all of the details noted above, plus the recommendations of the committee as to preventative measures to follow.)  
  • Contact the applicable union/employee group (ie. the same union/employee group of the individual who has been critically injured/killed)
  • Ensure that the College’s Emergency Response Core Group has been notified
  • Conduct an investigation; prepare and submit a final investigation report, including the details noted above, plus the recommendations of the JHSC as to preventative measures to follow, to the Ministry, on behalf of the College, within 48 hours.
  • Ensure that, in the case of a critical injury or fatality to an employee or student of a third party employer on campus (e.g. Aramark, Follett, SUCCI -- including students working on Ontario Work Study Program jobs for the College, etc.), that the third party employer on campus has been advised and that they are following up as required, further to the Occupational Health and Safety Act and Regulations.
