EMPLOYER CO-OP WORK TERM AGREEMENT: Completed by the Employer and returned to the college, prior to the commencement of work. Earliest work start date, first week of May;

STUDENT FINAL PERFORMANCE  EVALUATION (graded):  Upon receiving the above work term agreement, this document will be sent out through the Co-op Office to the Employer. Traditionally this occurs in August, unless a student leaves the workplace earlier than specified on the Work Term Agreement.
