Ski and Tubing Night Forms 

In order for members to complete registration for  Ski and Tubing night at Mount Baldy, the following forms must be completed by each member/guest who plans to attend and delivered to Sue Tucker in the SUCCI Office (Shuniah Building, Room 160) before February 12, 2020.

Form 1: Mount Baldy Liability Form (MUST be completed and sent to Sue)
Form 2: Mount Baldy Rental and Release of Liability (OPTIONAL - to be completed and sent to Sue if rentals are needed) - only top portion of the form, Height, Weight and Shoe size under the "SKIS" header, and page 2 (Release of Liability, Waiver of Claims) need to be filled out.
